First Visit at Rolle Oral and Facial Surgery

Upon your first consultation appointment at Rolle Oral & Facial Surgery, we have the capacity to proceed with surgery on the same day. However, in some instances, a second appointment is required for surgery, due to more complex medical conditions, for proper preoperative treatment planning.

You need to provide us with the following information at the time you are coming for a consultation:

  • The surgical referral form you have or an x-ray, if it is applicable
  • A list of All your daily medications
  • Do you have medical or dental insurance? If yes, then bring in the necessary completed forms so that it saves time for us, as well, and allows for proper processing of your insurance claim.
  • Also, you need to complete the Patient Registration Form Online before you visit.

Important

If the patient is a minor (under 18 years), they need to be accompanied by parents or guardians at the time of the visit.

If you are opting for IV anesthesia, it is mandatory for you to have a preoperative consultation and physical examination before the procedure. Don’t eat or drink anything eight hours before your surgery. You are suggested to be accompanied by an adult to drive you back home after the procedure is completed.

If you have any pre-existing medical conditions, like diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, and so on, you need to notify our office before the procedure. If you are currently on any medication like heart medication, anti-coagulant, or aspirin or any other, tell us about them too.

X-Rays

If you already have x-rays, as per the instruction of your dentist or physician, request them to forward the radiographs to us. Or if possible, bring the radiographs with you to our office. If you need any additional x-rays, we will obtain more in our facility.
